Children and Youth Services
Novak Center WIC (Women, Infants, and Children)
Provides nutrition education, breastfeeding education, a monthly food prescription of nutritious foods and other services for pregnant women and children younger than five years of age.
Golden Arrow Center for Mothers and Children
Services for mothers and children through kindergarten age who are in need. Provides such items as diapers, baby food, formula, children’s clothing, car seats, etc
Community Coordinated Child Care (4 C’s)
Assists parents in locating quality child care based on the children’s ages, days/hours of care needed, and preferred neighborhoods.
Safe Place
Provides emergency shelter for youth ages 12-17. They also have a lot of programs and lessons for youth.
Infant Resource Ministry
Provides baby clothing, infant formula, diapers, wipes, and other items for infants.
Counseling
Seven Counties
Provides telephone intake for first-time appointments and for on-going mental health counseling. Counseling centers are in several locations throughout Metro Louisville.

Emergency Financial Assistance
Community Ministries
Community Ministries provide assistance with rent, utilities, prescriptions, food, clothing and referrals to other community agencies. Assistance may vary among each agency. Not all services are available at each specific community ministry. Refer to the directory on the next page to locate the community ministry which serves the area of the person in need.
Metro Louisville Office of Social Services
The Housing and Rental Assistance program serves households with an employment history that meet eligibility criteria. Financial assistance must be used for rent, mortgage, deposits, and utility payments and households may apply only once in a 12-month period. Apply through the Neighborhood Place and call 502-574-4377 to determine the specific Neighborhood Place nearest the address of the person needing assistance.
LILHEAP (Low Income Home Energy Assistance Program)
Provides utility assistance for low income households in the fall and winter months. Call 502-574-4377 to determine when LIHEAP applications will begin this year. Signup for the regular component of LIHEAP for all low income households usually begins in October. The Crisis Component of LIHEAP for low income households with shutoff notices or overdue balances usually begins the first week of January and lasts until March or whenever the funds are depleted.

Dare to Care Food Bank
Dare to Care provides emergency food for persons in need. They have many distribution sites throughout Metro Louisville. To find the sight nearest the address of the person in need, hours of the site, and application process, call Dare to Care at 502-966-3821 or the specific distribution site, or visit the Dare to Care website.
Main distribution sites in Sojourn Midtown’s immediate areas are:

In addition to the main distribution sites, several zip codes have food pantries which may have a more limited amount of food available, as well as a mobile food pantry that makes periodic visits to several areas of Louisville.

Government Financial Programs and Benefit Programs
Social Security/SSI
Social Security Benefit programs are “entitlement” programs, based on your work history or your spouse or parent. Benefit types are Retirement, Survivor, and Disability. The benefit amount is based on your earnings or those of your spouse or parent.
SSI (Supplemental Security Income) is a needs-based program for people with limited income and resources. The benefit amount is based on Federal and State laws which take into account where you live, who lives with you, and what income you receive. Benefit types are Aged (65 or older), disability, and blindness. Some people receive both Social Security and SSI benefits.
Veteran’s Administration
To apply for VA pension and compensation programs, call 1-800-827-1000, visit at VAN website at benefits.va.gov, or visit the regional VA office
Medicare
Medicare is a federal health insurance program that serves people over age 65, younger disabled people, and dialysis patients.
Medicaid
Medicaid is a federal-state program that pays for health services and serves low-income people of every age. It is run by state and local governments within federal guidelines.
SNAP (Food Stamps)
Provides an EBT card that allows the purchase of nutritious food for most grocery stores. To apply, bring proof of identity, income, resources, and expenses as well as social security numbers of those seeking assistance.

Addictions Recovery for Pregnant Women
Centerstone Women’s Renaissance Center (WRC)
Women’s Renaissance Center, located in Shelbyville, is designed to serve pregnant women suffering from substance use and co-occurring disorders. The WRC specializes in providing a structured therapeutic environment that encourages growth, healthy coping skills, parenting education and a safe environment that establishes recovery and allows for the delivery of a baby free from illicit substances.
Project Link at Seven Counties Services
Project Link is a FREE program for pregnant women who are using alcohol and/or drugs. The program provides case management and peer support services that can help mother and baby connect with community resources, including finding OB/GYN care, transportation to doctor visits, housing assistance, mental health and addiction treatment services, Education on the use of alcohol and/or drugs, and assistance in obtaining baby items.
Intensive Outpatient Program features intensive outpatient sessions for pregnant and parenting women. Mothers are welcome to bring their children (up to 18 months) and feed or attend to them as needed.

Emergency Night Shelters for Homeless Persons
Coalition for the Homeless
The Coalition for the Homes provides centralized reservations of beds for the night and a centralized waiting list for housing for homeless families.
To secure a bed at any of Louisville’s emergency shelters for the night call 502-637-2337 from 10am to 4pm seven days a week. Homeless persons can go to the Coalition for the Homeless Office Monday through Friday from 10am to 1pm to reserve a bed.

The Center provides free, safe, confidential shelter for anyone who is at imminent risk of physical harm from intimate partner violence or sexual assault. Learn more about our emergency shelter here. If you are in need of shelter, please contact our crisis line at 502-581-7222.

Salvation Army Emergency Shelter
The shelter is a dormitory style facility with the capacity to house 98 men, 22 women and 6 families. Many of the beds are open and have no program requirements to stay. All bed reservations are made through the Single Point Entry 10AM-1PM to reserve a Bed - please call 502-637-2337.
Volunteers of America
VOA Home is dedicated to providing around-the-clock care and service to meet our community’s housing needs. Our comprehensive housing programs provide individualized case management to help unhoused individuals and families find safe, stable and affordable housing and give them the resources they need to live a self-sufficient life.
Wayside Christian Mission
Call the Single Point of Entry hotline at 502-637-2337 for a reservation, if you do not get a reservation from the hotline, it is recommended that you go to the shelter to see if any beds have become available. Re:Center’s Drop-In Day Shelter is a safe place to come during the day for homeless and vulnerable women and their young children (age 7 and under). While here, they may receive various services, love, care, and support in their unique set of circumstances.
Up for Women & Children (Women Only)
A drop in day center for unhoused women and their children and marginalized genders. UP provides comprehensive services: a safe space, shower, restroom, use of UP telephone, a mailing address, help obtaining vital documents, case management, and a place to connect with community resources and providers.
St. Johns Center for Homeless Men Emergency Day Shelter
Every day of the year, people experiencing homelessness find relief from the elements and a safe, affirming space at St. John Center. Since 1986, we’ve provided fresh coffee, hot showers, a place for guests to receive personal mail and phone calls, and access to needed supplies such as socks, gloves, and hygiene products, and that’s just the first step.

The Section 8 Rental Assistance or Housing Choice Voucher
Program is a federally funded program to provide finance assistance for eligible low income persons for decent, safe and sanitary housing. Application forms can be downloaded on the website or may be obtained at the agency office at 801 Vine Street.
Housing Group at Salvation Army
For many today, making ends meet is a daily challenge, forcing people to choose between things like heating their homes and putting food on the table. We understand the stress these situations put on families and are here to help. With utility assistance, our goal is to provide resources that will help families stay in their homes.
St. Vincent DePaul
To have a home, regardless of the square footage or closet space, offers you a place of refuge from the challenges of life. We don’t house those in need to get them off the streets, but rather, we house individuals and families to give them a better life. We average 400 people every night in our on-campus and off-campus housing programs, and we often go over the capacity of our bed count. To put that into perspective, there are roughly 165 cities in Kentucky with a smaller population size. Unfortunately, housing is a growing need in our community.

The Housing Partnership Inc. (HPI) counselors partner with you to create a personalized plan that improves your credit, sets financial goals, secures down payment assistance, and improves your housing options. You move toward homeownership on your terms and at your pace.
KY Housing Advocates
KY Housing Advocates was formed to help young adults in search of housing. KY Housing Advocates is dedicated to bridging the gap between homelessness and housing for young adults who have exhausted all other available resources. We are committed to providing transitional housing through a trauma-informed approach, advocacy, and case management. Our goal is to facilitate empowerment that leads to permanent self-sufficiency for homeless young adults ages 18-24.
St. John’s Center
St. John Center has accompanied individuals who formerly experienced chronic homelessness by providing long-term case management and support. With the help of regular in-home visits, mediation with landlords, assistance with job searches and disability benefits, and weekly life skills classes and community-building opportunities, those in the Permanent Supportive Housing Program can build a new life in their own home.

Christian Care Communities is Kentucky’s largest faith-inspired, non-profit provider of affordable senior living and long-term care for older adults. They offer independent living, skilled nursing, personal care, outpatient rehabilitation, & adult day health services.
Metro Louisville Housing Authority
The Housing Authority has available lists of rent-subsidize
apartment complexes for senior citizens and disabled persons in Metro Louisville.
Home Repair Programs
Metro Louisville Emergency Repair Program
The Home Repair Programs are committed to providing repairs to help stabilize the home for low- to moderate-income individuals who own and are currently living in single-family homes. These programs are funded by a U.S. Department of Housing and Urban Development (HUD) grant.
New Directions
Every June, New Directions conducts the Repair Affair for low income senior citizens and disabled persons in Metro Louisville. Applications are accepted from Jan. 1 through March 30. Examples of work done include exterior painting, yard work, door and window repairs, and building of wheelchair ramps.

Catholic Charities
Migration and Refugee Services (MRS) “welcomes the stranger” with open arms, helping those escapingpersecution begin new lives with dignity. Our multilingual, multicultural staff offers comprehensive services and begins assisting refugee clients pre-arrival by finding suitable housing and stocking the home with food and furnishings. Families are greeted at the airport and we work with them every step of the way to adjust to their new homes. This includes assisting with employment search and placement, school registration, English language training, and cultural orientation classes.

Legal Aid Society's mission is to pursue justice for people in poverty. We provide free civil legal help on issues related to income, family, housing, safety, and health.
Access Justice
At Access Justice, we seek to bridge the justice gap with compassionate legal service that depends largely on training clients to represent themselves. At our clinics, we provide the following legal services: Legal “Urgent Care” Services, Referrals to other Attorneys & Legal Organizations, Consulting with Nonprofit Organizations, Education Seminars, etc. Book an appointment for a clinic on the website.

Provides information and referral to community agencies and other services to crime victims.
Protective Services/Abuse and Neglect
Adult Protective Services
Services for victims of abuse, neglect, and exploitation for those who are age 18 and under.
Child Protective Services
The Child Protection Branch develops the Standards of Practice (SOP) state manual regarding child protective services intakes, investigations/assessments, and in-home ongoing cases based on the United States Code, Kentucky Revised Statutes, and Kentucky Administrative Regulations. The Child Protection Branch provides policy-driven guidance to Central Intake and field staff.

Senior citizen center serving Old Louisville, Downtown, Shelby Park, Smoketown and other areas with activities, nutritious lunch and social work services Monday through Friday.
Elder Serve
Provides many senior citizen services including adult day care, financial management, in home care services, Senior Companions and Telecare.
KIPDA Area Agency on Aging
Services for senior citizens and disabled persons with in home services, medical transportation and information and referral.
Louisville Metro Office for Aging and Disabled Citizens
Provides information and referral about community resources.
Senior Benefit Alliance
Assists with providing Medicare and healthcare assistance to seniors with a focus on low-income members of the community. Free benefits eligibility review.
Senior Nutrition Program
Provide Meals on Wheels and lunchtime meals at Congregate Meal Sites. Go to the website to fill out the referral form.
Transportation
Federated Transportation of the Bluegrass
Provides transportation services to medical appointments for eligible Medicaid recipients. Needs 3 business days notice to schedule. Cost – no charge with a valid Medicaid card
KIPDA Area Agency on Aging
Provides medical transportation for persons age 60 and over. Apply by calling the KIPDA office. Takes about 5 days for initial approval, then call well in advance to schedule rides. Cost – no charge.
TARC 3 Paratransit
Door to door service providing transportation for persons with disabilities who cannot use regular fixed route bus service to any destination in TARC’s regular bus route service area. The applicant must complete a written application form and have a medical provider complete a medical form. Forms may be obtained online or by calling TARC 3 office and requesting form be mailed. Cost - $3.00 for one way trip; $6.00 for round trip.
